Job Details:Role is located in Live Oak, FL in an office setting. Work schedule is typically Monday to Friday 8AM-5PM. Comprehensive benefit package offered. Competitive base pay.
Job Description Summary: Your responsibilities include developing, planning, implementing, and organizing clinical operations at our specific location. You will be responsible for observing and assessing the delivery and quality of client care and providing feedback and guidance to staff members to enhance performance and maintain standards of excellence. Role Description: Clinical Responsibilities: the assessment and reassessment of patients, including updating of care plans and interpreting patient needscaregiver assignments and oversee the delivery of care to all patients.case referrals.admission decisions based on patient needs and ensure compliance with admission criteria.supervisory visits with clinicians.quality improvement activities to address identified areas for enhancement.direct patient care in emergencies
Leadership Responsibilities: approximately 15 clinical staff, including hiring, training, and performance evaluation.assist and guide clinicians to enhance performance and ensure quality home care services.and ensure appropriateness of clinical documentation.documentation compliance with regulatory agencies and third-party payers.communication between team members, attending physicians, and caregivers.with the Branch Director and Area Director of Operations to establish revenue and budget goals.in sales and marketing initiatives.staff education based on clinical documentation review, trends, and feedback.situations, identify problems, and implement performance improvement initiatives.as Branch Director in their absence.in performance improvement activities and maintain ongoing clinical knowledge.relationships with referral/community sources and participate in professional organizations.Call After Hours rotation for administrative purposes.with weekend clinical manager coverage as needed.and manage CMS STAR ratings and HHCAHPS. Required Experience & Skills:year of recent experience as a Registered Nurse in home care environment.one year of recent experience in a supervisory role, preferably in healthcare settings.of an accredited School of Nursing.state license as a Registered Nurse.one year hands on OASIS experienceto effectively lead and motivate a team.of current CPR certification.driver’s license, auto insurance, and reliable transportation.
Preferred Experience & Skills:year of Homecare Homebaseyear of quality assurance experienceunderstanding of quality improvement principles and methodologies.
