CHC Mental Health Nurse Assessor (Outside IR35)
Eden Brown Synergy are actively recruiting for experienced CHC Mental Health Nurse Assessors for an opportunity we have available Outside IR35.
Our client are looking for someone to play a key role in supporting the Personalised Healthcare Commissioning service across the region. Working within the remit of the national framework for CHC and additional NHS funding streams, the successful candidate will deliver an effective and competent level of service and consistently deliver a patient-focused service.
Key Responsibilities:
- Managing a caseload of patients and contributing to performance improvement by taking a lead on identified areas where agreed.
- Assessing patients.
- Ensure that the national framework for NHS CHC and S117 entitlement is implemented and complied with.
- Taking a lead on complex cases, ensuring a high-quality of service is delivered at all times as well as supporting and mentoring staff to achieve continuous improvement.
- Managing a defined caseload for those with complex health needs.
- Ensure that principles and duties of safeguarding are consistently applied and met.
