About the Opportunity:Setting: Residential facilitySchedule: Monday to FridayHours: 7:30am to 4pm or 10am to 6:30pmPatient Population: Adult ID/DD OPWDD Patient Volume: Caseload of 20EMR: EpicResponsibilities:
The Registered Nurse (RN) will:Serve as the medical liaison for IDT team and ensure proper medical follow-upAssess injuries/accidents to ensure individuals receive proper medical careDevelop written plans for each person; modify nursing plan, as necessaryMaintain medical records for each individualAccompany individuals to medical appointments, as assignedEnsure proper medical follow-up through an on-call systemEnsures all medication is administered and stored, as necessaryComplete appropriate incident reporting paperwork, as requiredAct as a liaison with family members regarding medical issuesProvide ongoing training and supervision of AMAP certified staffPerform other duties, as neededQualifications:Graduate of an accredited School of NursingNYS Registered Nurse (RN) licenseSolid assessment, clinical, and documentation skillsPatient orientedGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired Skills:Bachelor's Degree in NursingOPWDD background
